Role focus - Post discharge working with adults 4-10 weeks post discharge to support assessment and support planning, MCA and safeguarding as appropriate - Also will be involved in CHC - Mix of office and home based due to community appointments Candidate profile - Social Work England registration or confirmed eligibility where the role requires qualified social work practice. - Relevant social care experience in a UK local authority, community, safeguarding or provider setting. - Right to work, professional references and role-specific compliance before placement. - Adult services, hospital discharge, assessment or community social care experience where relevant.
